Support Coordination

Support Coordination helps participants make the most of their funding through the National Disability Insurance Scheme (NDIS) and the Support at Home Program Australia.

This service assists individuals and families to understand their plan or package, connect with the right service providers, and coordinate the supports they need. Support Coordinators help organise services, manage service agreements, monitor budgets, and ensure supports are working effectively.

Support Coordination also helps participants build confidence and independence by guiding them through their funding, resolving issues with providers, and adjusting supports when needs change.

Our goal is to simplify the process so participants can access the right supports and live more independently in their community.
