It is the role of our support coordinator to work alongside you, your family and your service providers. We will help you understand your funding and entitlements and to ensire your support are set to best suit your needs.

We will help you with identifying your goals and connect you with what you need to achieve them and ensure the service work together smoothly to avoid overlaps or gaps. We will step in if there are issues with a service provider and check in the make sure the supports are working as intended and make any adjustments as they come up

In short, a Support Coordinator is like a guide or navigator—they don’t necessarily provide the direct care themselves but make sure all the pieces of care and support fit together properly for your wellbeing.
